Please print this form, fill it in and drop it off to the Lilydale Bowling Club. Forms are also available at the Club.

If no one is available to take the form, you are welcome to drop it into the Treasurers box, located just inside the red door.

If depositing with fees, please put everything together in an envelope clearly labelled with your name on the front.
